- Ovulation Process: Mittelschmerz is closely associated with the process of ovulation, which happens in the middle of the menstrual cycle. During ovulation, one of the ovaries releases an egg (ovum) into the fallopian tube, where it can potentially meet sperm for fertilisation. This release can cause temporary changes in the ovary, including the release of a small amount of fluid and blood.
