Mittelschmerz – literally 'middle pain' in German – could be behind those strange cramps that strike outside between periods. Never heard of it? Mittelschmerz might sound complex, but it refers to a relatively common phenomenon characterising pelvic discomfort, often referred to as ovulation pain – it can provide essential clues about your menstrual cycle. It occurs due to the process of egg release from the ovary, and its exact cause is not fully understood. Here's a closer look at what it entails: 

  • Ovulation Process: Mittelschmerz is closely associated with the process of ovulation, which happens in the middle of the menstrual cycle. During ovulation, one of the ovaries releases an egg (ovum) into the fallopian tube, where it can potentially meet sperm for fertilisation. This release can cause temporary changes in the ovary, including the release of a small amount of fluid and blood.
  • Follicular Rupture: The exact cause of mittelschmerz is believed to be related to the rupture of a mature ovarian follicle (a fluid-filled sac in the ovary). The pressure and stretching of the ovary during the rupture can lead to discomfort and pain in the pelvic area. This pain might occur either during the follicular rupture itself or shortly afterward.
  • Ovarian Changes: When the follicle ruptures, the egg is released, and the remaining follicular tissue transforms into a structure known as the corpus luteum. The corpus luteum produces hormones, including progesterone, which is important for preparing the uterine lining for potential implantation. These changes in the ovary can contribute to pelvic discomfort. 

While mittelschmerz is generally considered a normal part of the ovulatory process, the intensity and duration of pain can differ from one individual to another and from one cycle to the next. Pain on One Side

One hallmark of mittelschmerz is its one-sided nature. The pain is usually localised to the right or left side of your lower abdomen, depending on which ovary is releasing an egg during ovulation. This characteristic one-sided pain can help you differentiate mittelschmerz from other abdominal discomforts.

Mid-Menstrual Cycle Timing

Mittelschmerz typically occurs right around the time of ovulation, which is approximately in the middle of your menstrual cycle. Ovulation usually happens about 14 days before your next expected period, so if you're noticing the pain keeps coming back around this time, it could be mittelschmerz. 

While mittelschmerz can be harmless and may not require medical treatment, it can sometimes be mistaken for other pelvic pain conditions.
